Accreditation Statement

This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of the Institute for the Advancement of Human Behavior (IAHB) and Sanctuary of Endovascular Therapy. The IAHB is accredited by the ACCME to provide continuing medical education for physicians. 

Credit Designation Statement

The IAHB designates this live activity for a maximum of 19 AMA PRA Category 1 Credit(s)TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.  The Center does not endorse the off label use of any products for a purpose other than for which it was approved by the FDA. Conflict of interest disclosures are on file for all presenters.

Nurses

IAHB is an Approved Provider of Continuing Nursing Education by the UtUtah Nurses Association, an accredited Approver by the American Nurses Credentialing Center’s Commission on Accreditation. (UNA = 19 CEHs. Provider Code P09-03.)

Radiological Technologists

The 2012 SET Conference has been approved by the American Society of Radiologic Technologists (ASRT) for a total of 19.5 Category A credits. Attendees are responsible for checking in at the conference room entrance for each session. After the final day, a certificate of attendance will be provided by email for each registrant detailing total credits earned. All attendees must be present throughout the entire session to receive credit for that session. No partial credit will be awarded per session.
